There are 4 Sections separated by Light Blue Titles.

  • Section 1 is for your Employee information and you HR assigned Home Campus/Department. This is NOT for the campus you are requesting access to.

  • Section 2 is for Full-Time Employees ONLY. (If you are a sub or requesting access for a Sub, skip this section.)

  • Section 3 is for Long-Term Subs ONLY. (if the person you are subbing for is no longer a CISD Employee, please type “Vacancy” for first and Last Names.)

  • Section 4 is for the Type of Access being requested. If you need to know the types of permissions available, you can click the link in the last section to show you the list of available permissions.

    • Copy - If you are new or have a new position and need access copied from another employee, *choose Copy. *However, if you’re at a campus, this employee must be at your home campus and approved by your Principal.

    • Summer School - Request only during summer and must provide required information under “Other”.

    • Reinstate - If you had access and it expired, choose Reinstate previous years permissions, and new expiration dates will be applied.

    • Other - You can list campuses needing access to in the Notes Section under Other. Your campus principal (this form is automatically sent to your principal) CANNOT approve access to another campus. (If you are a District employee, SPED, T&L, etc., your district supervisor can approve campus access)

There are 2 Forms to choose from, Full-Time Access Request & Long-Term Sub Access Request, each has 3 required Sections to fill out. Each section is separated by Light Blue headers.

Once you have completed all questions in

each section on the appropriate form, “Submit” your request by using the button at the Bottom or Top of your Form.

If the form was Rejected or you need to make changes after it was submitted, use the “Recall “ button at the top, make corrections then Resubmit.
